. Late submission of work on a frequent basis
. Constant disharmony among team members, ie
- people are not speaking to each other
- increase in grievances
- complaints within a team
- evidence of conflict or hostility between team members
. Loss of production or unit output
. Confusion about assignments
. Unclear roles and responsibilities
. Directions misunderstood and not carried through
. Apathy or lack of interest or involvement of team members
. Lack of initiative, imagination and innovation
. Either high dependency on, or negative reactions to, the supervisor
. Complaints from outsiders about the quality of service and products
. "One bad apple" (eg lone wolf or rugged individual etc.) can ruin a team
. Too many meetings and too little action can squash motivation and dynamism
